Outpatient Care Management allows patients to receive expert medical attention and support services while continuing to live at home. It is flexible and patient-centered.

Post-Hospital Support
We provide follow-up care and monitoring to ensure a smooth transition from hospital to home, reducing the risk of readmission and promoting full recovery.
Social & Community Support
We connect patients and families to vital social services, community programs, transportation resources, and other non-clinical support that impacts overall health and wellness.
Care Navigation
Our care navigation guides you through complex healthcare systems, helping you understand your options, access services, and making informed decisions about your care.
Pharmacy Assistance
We help simplify medication management by offering support with refills, prior authorizations, cost-saving options, and education on how and when to take your prescriptions.
Convenient Home Care Services
Our skilled providers deliver personalized and compassionate care to patients in the comfort of their own home, which is particularly beneficial for mobility challenged or complex conditions.
Who is Eligible
Our outpatient services are available to patients who meet one or more of the following criteria:
✓ Recently discharged from the hospital or emergency department
✓ managing one or more chronic conditions (e.g., diabetes, heart failure, COPD, chronic kidney disease, asthma)
✓ at risk of frequent hospital visits or readmissions
✓ experiencing barriers to care such as transportation, housing, or access to medication
✓ referred to by a healthcare provider, case manager, or social worker
✓ covered by participating insurance plans or eligible under community-based programs
Additional Eligibility Notice:
Participation in the program is limited to patients enrolled in certain coverage plans. Please consult with your care team to verify eligibility.
